The Community Connector Programme connected with the Integrated Care Hub for the Older Person (ICPOP Dublin South East) is a specialist social prescribing service with a focus on people over 65 who have more complex care needs.

This pilot programme which was set up jointy and locally by Southside Partnership DLR in collaboration with the HSE in April 2023 is now an integral part of the Integrated Care Programme for Older Persons (ICPOP) Service. The Community Connector service currently covers selected areas in West Dun Laoghaire-Rathdown and South Dublin City and takes its referrals directly from the Integrated Care Team.

The programme ensures a further step to providing personalised health and wellbeing plans to our older population to reduce unnecessary presentation to unscheduled health care. Upon completion with the Community Connector Service, clients can be referred back into the community to other community organisations and services that are available across Community Healthcare East.

The Community Connector role centers around supporting older individuals who may have difficulties accessing activities and interests that they may have previously enjoyed or have always wished to try but never have.

Having a specialised social prescribing service for older community members linked directly with the Integrated Care Hub is providing a non-medical intervention straight after discharge to support people to stay well in their own community, discover what is available and encourage new connections along the way.

Social Prescribing is about supporting the health and wellbeing of people by helping them to connect with local, community-based activities as well as national and online supports. The DLR Social Prescribing Team will talk to you about what activities interest you or what supports you may need and will then assist you in availing of these activities.

The HSE Living Well with Dementia (LWwD) programme is an award-winning initiative which develops and delivers innovative services and supports to people living with dementia and their families in their communities. LWwD is a community-based programme which focuses on the strengths and personhood of the individual. Our vision is of a community that respects, supports and empowers people with dementia, a community in which people with dementia are socially and culturally valued, and where they can actively participate in society in a safe environment. The Healthy Food Made Easy (HFME) Programme is a community based basic nutrition and cookery course funded by the HSE and supported by HSE dieticians. The course helps people to change to a healthy diet, plan meals on a budget and make easy to cook meals. The course content is based on the most up-to-date healthy eating guidelines from the Department of Health.

Our SWAP programme is funded by Healthy Ireland and SICAP (Social Inclusion Community Activation Programme) and supports community groups of older people and people with disabilities to take part in classes that promote positive mental health and good health and wellbeing in their local community.

The S.A.V.E. (Southside Addressing Violence Effectively) Forum is an interagency forum of organisations and workers in the Dun Laoghaire Rathdown area who work directly or indirectly with people experiencing and surviving Domestic Abuse in all it’s forms. The Forum works with all relevant bodies to develop a comprehensive response to Domestic Abuse and Violence. The group raises awareness of the effects of Domestic Abuse within communities. It works to establish effective links with agencies working on behalf of those at risk of Domestic Violence.
